I have taken a couple months to write this as I don’t like leaving bad reviews BUT I want to save other people. My dad stayed at this facility for rehabilitation, and the experience was far worse than I ever could have imagined. I feel obligated to warn other families. On multiple occasions, my dad called me in severe pain. This guy had a heart attack and told me his pain was at a 4 so I knew it was bad.When I arrived, his foot was so infected it was making him vomit. Staff repeatedly dismissed it as “constipation,” rolled their eyes when I asked for him to be sent to the ER, and showed no urgency despite obvious signs of infection. When he was sent to the ER, he would return missing personal items — including his wheelchair and even cash. Despite the fact that he is not supposed to walk on his diabetic wounds, no one treated the missing wheelchair as urgent. Several times I found that his wound care had not been done at all. A supervisor confirmed that the LVN had charted it as completed, but the bandage date was three days old. This wasn’t a one-time mistake — it was a pattern. The facility also dropped his third‑party insurance without notifying us, which created even more unnecessary stress. There were days he didn’t receive full meals, and when I brought this up, the head RN made me feel like I was imagining things or that my dad was lying. Instead of addressing concerns, they dismissed them. Towards the end of his stay I was told he would be temporarily switching rooms for some painting. I went to visit again and he was put into a make shift room in the back with two other clients and no T.V. When I asked how long the renovation would last and when he would be in his old room they informed me the transfer was permanent. To make matters worse, the administrator told me my dad was “cleared by a doctor” and that his wounds were “all healed.” When I brought him home, he still had two large diabetic wounds on the bottom of his foot. Nothing about that was “healed.” I didn’t realize how unacceptable this place was until he transferred to another skilled nursing facility. The difference has been night and day — attentive staff, clean environment, actual communication, and genuine care. My dad is finally safe and supported. I would never recommend this facility to anyone. Please think twice before sending someone you love. Here is a photo of the day I brought him home when he was “healed”.

Good clean facility nice staff but they should feed their patients more when they ask because they are still hungry. My sister was in here after breaking her hip and I would always ask staff to get her more food due to her being hungry but when they had pizza they would only give her one piece of pizza and she would continue being hungry.
I was there for 5 months. Staff was AMAZING! The food was the best! The biggest shout out to therapy department and nursing staff for making me better!!
It’s great that a facility like this exist! Clean and friendly facility with amazing staff. There are private rooms, semi private and even the three bed room are spacious Great nurses and approachable staff makes for a speedy recovery. They specialize in wound care, respiratory and rehabilitation. The therapy room is the largest in Porterville and is top notch. I would feel comfortable putting mom here. Sequoia transitional care is the best
Treated very well. Any problems I had were resolved in a timely manner. And the food was much better than the previous care home I was in.
While maintenance repeatedly attempted to resolve the problem with the toilet not properly flushing for over two weeks in my mother’s room, the issue finally came to a head when the toilet began leaking all over the floor and numerous unsatisfactory explanations were offered by several staff members. Their solution was to post a sign saying don’t use the toilet and instead use a portable bucket and empty sewage in shower drain. Now that a few more weeks have passed the situation has improved and staff frequently attempts to resolve the facility wide systemic plumbing concerns.

My mom was in this facility for a few months after being in a bad accident. The staff was all very nice gentle and very very patient with her. We would be there late at night and the staff was always very understanding and never told us anything. Would definitely recommend this place to anyone needing care.
I’ve been here a month… the staff is great, many going above and beyond. The food is terrible, but that’s the same everywhere. I am expected to be here another 2-4 months, so I will update. I will say that I have a private room which makes a difference, but every time I’ve needed something, I’ve been able to bring it or they have supplied it. They even let my kitty visit on weekends. I will continue to do monthly updates as along as I’m here, but so far, so good.
Staff is amazingcaring n always so atentive
Sequoia Transitional Care was very welcoming during my mom’s stay there. My mom suffered serious injuries and had to be placed in sequoia transitional care for about 2 months and she absolutely loved it there! I would visit my mom every other day and she said she felt like she was at home because CNA and staff would come in to check on her and talk to her every so often! I definitely recommend for anyone who is looking for a place like this for a loved one.
This place was very welcoming from the moment we walked in. My mom was there for over 2 months and almost all staff, nurses and CNA’s made her feel safe and comfortable. Was very hard to bring her home because she got use to the nice hospitality environment there.
Yes, I would like to share my experience. This is at this facility which has been unbelievably. Good came in here, not knowing anything. Could not talk. Could barely just came out of a coma. I didn’t realize anything that was going on and all the CNA’s and the nurses Were tremendous. I can’t say How clean this establishment is and how they work with you. I’ve been to other establishments with friends and this stuff. You like this? This is been a very big eye opener and I am very grateful that I was able to be here with them. I will miss them because I am going on well. Thank you again. Bye.
My dad was there for 1 day and a half and had to be rush to the ER as his sugar level went down to 44, and staff there said it would be okay, but the after several hours it went up to 105 or so my dad looked bad, he was like incoherent and breathing very heavy and nurse would say he will be okay. Finally we/family had to tell them to call the ambulance and take to hospital and thank God we did because who knows what might have happened. So facility seemed nice but I do not know about the care. Now after being in the hospital for a few days we had no choice but to take him back to this place and I hope pray staff pays more attention. I wish the people that work there work there because they love they chose to do not just to have a job. Please show you care, I didn’t want to do any star rating but I had too to post this review but time time tell if I can see good things to give a better rating.
When presented with a problem, staff does not care. When approached they become very rude
This facility is very well kept and the staff are always friendly. With a new look and great staff, this is the perfect place for rehab and nursing services!
Facility is great, however management need some serious help.
My friend’s mom was here and was treated very badly they kept her on sleeping meds would not call 911 for her and had her benefits switch from her direct deposit to paper check had it sent to there address and deposited it in there account.. her insurance paid for her stay so what did they do wit her money

horrible place to put your family member we were not notified our family member had died we found out when the funeral home called. they had no issues calling us for other things, very unprofessional.
This place looks like its falling apart.
